Overview

We are looking for an Clinical Practice Manager to join our team. This position will directly report to the Assistant Director PC, Branch Director PC or Branch Manager PC and is responsible for the branch management of resources, care plans, and care coordination that will ultimately impact and/or influence patient outcomes and branch budget goals. Provides oversight for appropriate and compliant clinical episodic reimbursement. Works under general direction through established performance criteria. Will be monitored and measured under general supervision of the Executive Director.

• Has oversight of clinical activities, including patient assessments, individualized care plan development, care coordination, service level determination, on-site field visits, and the implementation/coordination/maintenance/evaluation of care plans to promote, maintain and restore the patient’s health.

About You

• Associate’s Degree in Nursing from an accredited school of nursing required; Bachelor’s degree in Nursing preferred.
• Current State licensure as Registered Nurse.
• OASIS certified or will be required to obtain OASIS certification within 1 year of hire as a MCP
• Minimum of three years clinical home care experience
• One year in a clinical management role preferred
