Health Navigator Advocate – Bilingual Mandarin

The Mandarin-speaking Health Navigator Advocate functions as an employer-based, non-clinical liaison between the client’s onsite health center (and affiliated clinicians) and the client population.

• Increase the utilization of the onsite health centers, improve the proper use of the client’s network and benefits, and the appropriate utilization of available resources.

• Builds awareness of the Medical Centers and healthcare resources within all client locations and facilities.

• Facilitate appropriate utilization of available resources by educating, engaging and activating employees to achieve health improvement and wellness goals via zoom, line-up presentations, onsite presentations, one on one meetings, group events, etc.

• Facilitates onsite wellness initiatives (flu, covid, shingles vaccines, etc.) at various health center locations. Prepares and provides marketing materials, flyers, emails, brochures, etc. outlining services available to employees at wellness center locations.

• Identifies employees with disease, pre-medical conditions, illness related profiles and refers employees to eligible programs treatment and disease management programs, such as the telephonic Personal Health Team, Case Management, weight management, mental/depression and other applicable resources.

• Researches and responds to health information and wellness plan inquiries.

• Completes day-to-day Health Engagement Services tasks without immediate supervision, but has ready access to advice from more experienced team members.

• Tasks involve a degree of forward planning and anticipation of needs/issues. Resolves non-routine issues escalated from more junior team members. Provides follow up and follow through services for referral management, preparing patients for specialist visits, acting as liaison between patient, family, specialist/ providers and social determinants of health resources

• Builds awareness of the Health Center and resources within all client locations – will need to travel to multiple sites and conduct individual and group promotional events to educate on scope of services available. Leave behind promotional material posted onsite as allowed/appropriate.

• Educates customers on the proper use of their Network and Benefits to achieve best outcomes at minimum cost, up to, and including, selecting specific Providers and Facilities for their healthcare needs.

• Acts as Network Ambassador on behalf of the health center, building relationships with preferred local physician groups to facilitate priority appointment availability and patient information exchange, coordinating efforts with account teams/Ops Directors

• Partners, and functions as central conduit between employer, provider (PCP, Onsite Health Center) and all available programs and resources.

• Maintains documentation activity in Outlook and Excel, to be shared with internal resources as well as Client.

Qualifications:

• A minimum of a BA/BS required with emphasis in Wellness and Health Promotion.
• At least 3 or more years of experience in a clinical practice or health promotions role
• Bilingual – Mandarin is required.
• Strong Computer skills (specific software e.g. Outlook, Word and Excel)
• Must be able to work both independently and in a team environment
• Must have flexibility and enthusiasm in learning new information and developing new skills quickly.
• Comfortable communicating with multiple clinical partners
• Experience working directly customers/patients; excellent customer service skills
• Excellent oral and written communication skills

This role is based on-site in Phoenix AZ.
