Pay Range
(RPN) $22.44 – $28.99 / (RN) $28.69 – $36.71
The Palliative Care Program is dedicated to delivering comprehensive home nursing services tailored to meet the unique needs of individuals, families, and groups facing end-of-life circumstances. Our team of skilled nurses is committed to elevating the standard of care for palliative clients, with a primary focus on proficient symptom and pain management. Through our specialized services, we aim to enhance the overall quality of life for individuals receiving palliative care.
Our dedicated nurses operate in the North York and Durham region, providing compassionate and personalized care within the comfort of clients’ homes. Please note that this is a visiting nurse opportunity, regular travel and driving are integral components of the position.
The Palliative Care nurses will provide palliative assessments and end-of-life care to patients. Some of the nursing services that you will be providing include wound care, IV therapy, injections, removal of PICC lines, chemo disconnections, and more. Nurses will attain specialized clinical skills in (Patient-Controlled Analgesia (PCA) pumps, pain management, and symptom management.) Additionally, conducting mental health support and check-ins with clients are also required.
There is a $2000 Sign-On Bonus for full-time nurses in Durham!
Key Areas of Accountability
VHA’s Nurses are required to do the following tasks, but not limited to:
Assess, Monitor, and Makes Care Planning/Care coordination
Evaluates care plan interventions through on-going reassessment and evaluation results.
Ensures the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
Demostrates professionalism, leadership, judement, and accountability in interactions with client and families.
Supports client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Informs nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
Maintains documentation standards according to the College of Nurses of Ontario.
Required Qualifications:
Registered and in good standing with the College of Nurses of Ontario (CNO).
Membership of your Professional Association preferred (RNAO/WeRPN).
RN diploma/degree or RPN diploma from a recognized university or college.
Valid First Aid or CPR Certification.
Preferences are given to candidates who have completed their Fundamentals of Palliative Care, LEAP, and/or CAPCE courses or work on these certificates within 1 year of hire with VHA.
Acute Care and Medical-Surgical nursing experience is an asset.
Knowledge of community nursing is an asset.
Demonstrate effective communication (written and verbal) and documentation skills.
Demonstrate reliable attendance, punctuality, and availability for the role.
Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is required.
